1. Your referral

Bring a current referral from your GP or specialist. Referral validity and Medicare eligibility should be confirmed with the practice before your appointment.

2. Preparing for your consultation

Please bring a medication list, relevant reports, recent results and the contact details of health professionals involved in your care. It may also help to note the main concerns or questions you would like to discuss.

3. Privacy and coordinated care

Your consultation is confidential. Clinical information may be communicated to your referring doctor or other treating professionals when appropriate to your care and consistent with legal and privacy obligations.

4. Appointments and cancellations

Patients are responsible for keeping track of appointment dates and contacting the practice as early as possible if a change is needed. Ask reception for the current cancellation policy.

5. Prescriptions, letters and forms

Appointments are required for prescriptions, medication changes, medical certificates, letters and clinical forms. Routine email is not a substitute for a consultation.

6. Email and urgent concerns

Routine email and phone channels are not emergency services. Do not send urgent clinical information by email. If you are in immediate danger, call Triple Zero (000).
